In-Home Senior Care and Geriatric Care Management

Geriatric Care Management and Consulting

Our registered nurse will complete a geriatric care assessment to evaluate medical, physical, financial and spiritual needs. She will assist to facilitate the necessities required for functional living, or help to achieve placement into an Independent Living Facility, Assisted Living Facility or Nursing Home. The fee for this service will be waived if the client initiates home care with our agency.

 

Home Care

At Caring Connection, Inc. we have three levels of care determined by a scoring index of client needs, and assistance with activities of daily living.

Level I Companion Care

Basic assistance with activities of daily living and companionship will be provided. Some or all of the following may be required:

  • Companionship
  • Cooking/meal preparation
  • Light housekeeping:  Bathroom, kitchen, floors, and trash removal
  • Laundry
  • Out of home activities
  • Monitoring for wandering
  • Care transportation
  • Physician appointment and errand accompaniment

Level II

Level II services are a slightly higher level of care. Our RN will complete a nursing assessment and develop a plan of care. Thereafter, our RN will perform an on-site supervisory visit of the caregiver to ensure care plan adherence every 90 days .

Care may require some or all of the following:

  • Showering/Bathing/Sponge bathing
  • Dressing/Grooming
  • Ambulation assistance (cane, walker, wheelchair, scooter)
  • Toileting
  • Monitoring fall risks
  • Feeding
  • Level I services

Level III

Level III services offer the highest level of care provided to clients.  Our RN will complete a nursing assessment and develop a plan of care. Thereafter, our RN will perform an on-site supervisory visit of the caregiver to ensure care plan adherence every 90 days .

This client may require some or all:

  • Incontinence care
  • Bed bound assistance
  • Wound care
  • Alzheimer’s and Dementia care
  • Transfer assistance or Hoyer lifting
  • Colostomy care
  • Hospice care
  • Medical device and supply ordering
  • Level I and II services

Welcome Home Program

Hospitals have found that patients that get discharged with minimal help and support at home are at higher risk for readmission into the hospital. At Caring Connection, Inc. we help our patients stay at home after an inpatient stay. Our caregivers will meet you at the hospital or other facility, transport you home if necessary, obtain all medications and supplies needed for recovery, assist with home set up and reacquainting into the home setting. This is a 6-hour program but can be customized to fit your needs.

In-Home Physicians

Caring Connection, Inc. is also affiliated with home care physicians and podiatrists. If you are unable to travel to your appointments, we will set you up with our physicians to have them come to you and treat you in the comforts of your home. We will correspond with them to ensure continuity of care.
